Terms of Service - Clinic

This document is supposed to outline the rules and regulations that outline the service that is provided in the premises of eDental-Clinic hereby referring to the mobile clinic and the office located next to it at Clonshaugh Technology Park D17E782, Dublin, Republic of Ireland. This document will outline the scope of the service that the patient awaits upon entering and visiting the dentist.

Refunds

We don't take responsibility for damaged items of any sort, where the damage was caused by the customer or through not proper behaviour of the customer. The dentist will provide with proper recommendations and aftercare-advice which -when properly followed- ensure the treatment achieved the anticipated outcome.

Consultation fee

Your first consultation with us incurs a fee of 40 Euro which is deductible of your final bill, if you proceed with your treatment. If you fail to attend an consultation without prior 24 hour notice then we will reserve ourself the right to keep the fee as compensation. You can reschedule twice, if you however don't attend the consultation after rescheduling it twice, the fee will be kept as well. For every further consultation you will be charged another fee of 40 Euros. If you cancel the appointment properly by contacting our team by phone or message support on time, the fee will be refunded. However we reserve ourself the right to keep a processing fee in the maximum amount of 5 Euro. The only accepted form of payment, is either by debit card or by electronic funds transfer. No cheques or American Express credit cards are accepted.

Feedback, compliments, suggestions and complaints

eDental Clinic is a technology company, which strives for innovative, intelligent solutions. Occasionally people have different perspectives. Every thought counts and we may use your feedback to enhance the service provided to you.

a) If you have a matter to raise, regarding any aspect of your care, please let the Practice Manager know. We’ll do all that we can to resolve it to your satisfaction, both promptly and professionally. The Practice Manager is the person responsible for looking into and responding to all patient complaints.

b) If the matter relates to clinical dental treatment, the Practice Manager will always seek the treating dentists’ comments in order to provide you a full detailed response. Please note we will refer to the consent forms which are the legal basis of the terms of treatment and the service we provide. We will also draw on any meta data captured in the app to compile and encapsulate a full 360 view of the matter.

c) If complaining on behalf of someone else, please note, we will require written consent in order to discuss their personal information. We cannot accept a message or a call from someone claiming to allow them to act on your behalf.

d) We’ll acknowledge your complaint in writing within 30 days and will also include a copy of this complaint procedure. We will look into the matter and aim to provide a full response within 30 working days. If, for any reason, we are unable to complete our findings within 30 working days, we will notify you, giving reasons for the delay and the likely period within which the findings will be completed.

e) If you’re not satisfied with the outcome received, please let the Practice Manager know. In some circumstances, this may be escalated to our patient liaison team to review and provide a final response.

f) If you remain unhappy once we have provided our final response, you may be able to refer your complaint free of charge, for an independent review. Please note that time limits may apply, so you’ll need to get in touch as soon as possible. We are licenced, indemnified and regulated professionals and as such conform to a code of practice. We will remind all patients to exercise patience and reciprocate respect to our staff.

Consent forms

Consent forms cover the basic procedure and the resulting recommendations from the dentist. The terms outlined in the consent form are the legal basis for which we carry out our service. Please take note that this is a legally binding document between you, the patient and the dentist at eDental-Clinic. By signing this you consent to the treatments outlined in the consent form.

To request access to personal data, you can fill out the submission of data form in our app or request by email with your name, signature, reason for data request, proof of identity signature and proof of address.
